In Time of Bereavement <\/h2>\n\n\n\n

Death is an inevitable fact of life. However many of us never think about what we need to do until we are faced with the situation. It is at this time you need all the help and support possible to help you through the grieving process. We at the surgery are available to give you advice and guidance: However, there are certain practical steps you might need to know about. These are things that you will have to do:<\/p>\n\n\n\n

If Someone Dies outside of hospital<\/h2>\n\n\n\n

The GP will be responsbile for producing the cause of death certificate (MCCD), to do this we need to refer the death to the Medical Examiner. The Medical Examiner is a recent, national government program, which has been set up to answer questions you may have about the events surrounding the death. They will try to help you understand what happened and why. They are also able to raise any concerns you may have about patient care and communications. To enable us to refer we will need the following information: <\/p>\n\n\n\n

Nominate one person to be the practice contact, we will need the mobile number and email address, this person will also be contacted by the Medical Examiner<\/p>\n\n\n\n

The funeral director you are using <\/p>\n\n\n\n

Whether the deceased is to be cremated<\/p>\n\n\n\n

If the death was verified by the amublance service we may ask for a copy of the yellow form that will be left in the home <\/p>\n\n\n\n

The medical examiner will advise the GP when the death certificate can be produced, we will contact the nominated person to inform them and it will be sent to the Registrars office electronically, you do not need to collect from the surgery. Once we contact you, you can then book an appointment to Register the Death, by calling 0300 790 0314 from 9am to 5pm Monday to Friday or you can book an appointment online at https:\/\/m.luton.gov.uk\/Page\/Show\/Community_and_living\/register-office\/Pages\/Deaths.aspx#death<\/a><\/p>\n\n\n\n
